VA Transforms End-of-Life Care A monograph on VA's recent transformation in care at the end of life has just been released, describing the advances in access and quality through VA's bold changes and enhanced collaborations with community hospice partners. This report includes stories from veterans of what this care means to them and is being distributed to VISN and facility directors as well as to Hospice Points of Contact throughout VA. Titled, "VA Transforms End-of-Life Care for Veterans," the report is published by VA with support from the National Hospice and Palliative Care Organization and Ethos Consulting Group, LLC.
Authored by Larry Beresford, a freelance writer based in Oakland, CA, the monograph describes current VA initiatives in end-of-life care; benefits, policies and services for enrolled veterans; partnerships with hospice at the local, state and national levels; and important differences that providers should be aware of as they individualize end-of-life care for veterans. Most of all, the report celebrates the remarkable growth of hospice and palliative care services for veterans and the emergence of productive partnerships between VA and community providers. It also calls on hospice agencies and VA staff to partner to assure that we honor veterans for their service and respond to their unique needs, wherever they receive their care.
