Colorado Bereavement Services Project Description and Forms
Thanks for your interest in the Colorado Center for Hospice & Palliative Care “Hospice Bereavement Manager Software”. This software was developed over the course of several years, sponsored in part by a grant from The Colorado Health Foundation (formerly: HealthONE Alliance) located in Denver, Colorado. Information regarding the grant (Colorado Bereavement Services Project) is located in the software manual on the CD.
The Hospice Bereavement Manager Software is individually licensed (i.e., multiple hospice locations would need to purchase a license per location). Pricing was set to be affordable for smaller hospices, yet equitable for larger hospice organizations.
The software was created to assist hospice bereavement programs in three ways:
- Tracking bereavement clients.
- Developing and following individualized care plans, including risk assessments at 6-weeks and 13 months, for bereavement clients.
- Researching risk assessment data to better understand and enhance bereavement services.
- Computerized tracking of bereavement client information.
- Computerized ability to update and print bereavement client individualized care plans and risk assessments.
- Mail merge templates for regular mailings.
- Tracking group participation.
- Tracking staff and volunteers assigned to bereavement clients, as well as referral sources.
- Computerized contact schedule (i.e., a calendar of scheduled follow-up calls).
- Tracking multiple bereavement clients for an individual's death, as well as multiple deaths for an individual bereavement client.
System Requirements:
- Microsoft® Windows 98, Windows ME, Windows 2000, or Windows XP operating system.
- Microsoft® Access, versions 2000, XP, or later.
- 20 MB of available hard disk space.
- Internet access and an email address (to participate in the research arm of this software, and to receive information / updates regarding the software from CHO).
- By default, this program will be loaded into directory C:CHO and will create this directory if needed.
Technical Assistance:
Technical assistance is available for this software by contacting the Center. The first 30-minutes of assistance is provided free of charge, additional assistance is available at a fee of $40/hour.
Software Updates:
No future software updates or new versions of the software are planned at this time.

- Marcia Lattanzi Licht, MA, RN, LPC – Principal Investigator of the Colorado Bereavement Services Project and Professional Education Specialist for the National Hospice & Palliative Care Organization. Marcia, along with consultation from J. William Worden, PhD, guided the theoretical development and bereavement expertise for this project.
- Julie Thomas, MA, LPC – Grief Services Coordinator, HospiceCare of Boulder and Broomfield Counties. Julie, with technical assistance provided by Ko Clifton, developed and modified the final software and training manual. Her contributions to this project cannot be understated.
- Kim Mooney, BA, CT, CTR – Grief Services Manager, HospiceCare of Boulder and Broomfield Counties. Kim chaired the Colorado Hospice Organization Bereavement Peer Group during the entire development and conclusion of this project. The Bereavement Peer Group reviewed numerous drafts of research instruments and early versions of the software.
- Andrew Gebbie, Billing Administrator, Hospice & Palliative Care of Western Colorado. Andrew created and updated the initial software for this project, and thereby offered a concrete product to develop our theoretical concepts.
- Al Canner – Al was the Executive Director of the Colorado Hospice Organization and worked collaboratively with Marcia Lattanzi Licht and the peer group to launch this project.
- Darla Schueth, RN, MBA – Executive Director, HospiceCare of Boulder and Broomfield Counties. Darla permitted CHO to use and further develop the bereavement software Julie Thomas and Ko Clifton created for the purposes of this project.
